About The Position
BrightStar Care, a leader in home care, is currently seeking compassionate and dedicated Licensed Practical Nurses (LPNs) or Registered Nurses (RNs) to join our team. This is your chance to make a significant impact on the lives of patients and their families, providing care that truly makes a difference! Traveling to a client’s home within our territory is a requirement. We are just expanding to the St. Cloud Community.
Requirements
- Valid LPN or RN license in the state of practice. (Minnesota)
- Experience in home health care or similar settings is preferred.
- Strong interpersonal skills and a passion for providing empathetic, respectful care.
- Ability to work independently and efficiently with keen attention to detail.
- Ability to Travel and Commute throughout Territory (Maple Grove, Elk River, Anoka, Circle Pines and Surrounding Areas)
- Reliable Vehicle, Current Auto Insurance, and Valid Driver's License
Responsibilities
- Provide nursing services to patients in a home setting.
- Services may include, but not limited to; wound care, INR, Medication Management, Catheter Cares, etc.
- Perform routine nursing care to patients as prescribed by the healthcare provider.
- Monitor patient health and maintain accurate records of vital signs and health progress.
- Educate patients and their families on health maintenance and disease prevention
- Document Nurse Visits in accordance with State and Joint Commission Guidelines.
