About The Position

Lehigh Valley Health Network (LVHN) is seeking a full-time Home Care Triage Nurse to provide clinical support to the clinical services team. This role assists in coordinating patient care services by performing phone triage, data/order entry, patient/staff scheduling, and other clinical tasks. The territory for this position covers Bucks, Montgomery, and Chester counties.

Requirements

Specialized Diploma from an NLN approved/accredited nursing program.
1 year of nursing experience in a healthcare setting.
1 year of direct care nursing experience.

Demonstrates analytical and problem-solving skills.
Ability to perform duties with accuracy and attention to detail.

Knowledge of agency and network clinical policies, procedures, and resource materials.
BLS - Basic Life Support AHA - American Heart Association certification required within 30 days of hire.
RN - Licensed Registered Nurse in the State of Pennsylvania required upon hire.
Valid Driver's License for Pennsylvania or New Jersey required upon hire.

Nice To Haves

Associate’s Degree in Nursing or Bachelor’s Degree in Nursing.
1 year of home health/hospice experience.
Less than 1 year of nursing experience in a clinical office setting.
Less than 1 year of nursing experience performing phone triage/on-call responsibilities.
AMB-BC - Ambulatory Care Nursing-Board Certified - American Nurses Credentialing Center within 3 years.
CHPN - Certified Hospice and Palliative Nurse - Hospice and Palliative Nurses Association within 3 years.

Responsibilities

Triages patients through phone calls utilizing critical thinking skills and sound clinical judgment.
Performs data/order entry, patient/staff scheduling, and other clinical tasks related to the provision of patient care.
Provides hand-off communication to members of the care team as appropriate regarding triage information, patient status updates, and patients who require a follow-up visit or phone call.
