About The Position
Apex Rehab Group is growing and looking for Occupational Therapists and Physical Therapists to join their home care team. This role is ideal for individuals who enjoy working one-on-one with patients, have flexibility in their schedule, and want to make a real difference in patients' lives. The position is located in Sullivan County and offers full-time, part-time, and per diem options with the ability for therapists to set their own hours.
Requirements
- Licensed OT, PT, or ST in New York
- Reliable transportation and a valid driver’s license
- Good communication skills and a positive attitude
- Ability to manage your own schedule
Nice To Haves
- Home care experience is a plus, but not required
Responsibilities
- Provide therapy services to patients in their homes
- Create and carry out personalized treatment plans
- Track patient progress and adjust care as needed
- Educate patients and families
- Complete simple, timely documentation
- Work independently with support from our clinical team
