About The Position

Join Addus HomeCare as a driven, organized, and compassionate Service Coordinator to lead the charge in scheduling caregivers, ensuring top-quality service, and conducting in-home visits. This role involves juggling schedules, supporting field staff, handling client updates, and stepping in to keep care plans on track. The ideal candidate thrives in a fast-paced environment and is passionate about making a difference in the lives of older adults and individuals with disabilities, enabling them to live safely and independently at home. The position is full-time, Monday through Friday, 8:00 am to 5:00 pm with a 1-hour lunch break, with evenings and weekends as needed. The location is Addus HomeCare, 1240 N. Riverside, Medford, OR 97501. This role is responsible for scheduling and supervising in-home care workers and clients in a geographic area, offering the satisfaction of helping vulnerable populations live safely at home.

Requirements

  • Must have high school diploma or equivalent.
  • 3 to 5 years of Industry experience required.
  • Interpersonal, organizational and communication skills.
  • Computer skills including but not limited to Microsoft Word, Microsoft Excel and Scheduling program.
  • Must have DL to complete in home supervisory visits.

Nice To Haves

  • Supervisory and/or home care experience preferred.

Responsibilities

  • Creates work schedules by entering schedules into the system, manages changes to client schedules due to client request, illness, vacation or leaves of absence.
  • Provides alternate coverage to ensure the client�s care plan is followed and client services are not interrupted.
  • Contacts care providers and clients to provide service updates.
  • Conducts monthly client wellness calls and conducts home visits as required.
  • Provides thorough, complete follow-through on escalated client complaints and theft claims.
  • Supervises direct service employees by setting expectations for attendance, performance and conduct by holding employees accountable to the company�s policies and guidelines.
  • Ensures the appearance of the branch�s open environment is professional: neat, clean, orderly and generally free of clutter.
  • Maintains a high degree of confidentiality at all times due to access to sensitive information.
  • Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the agency.
  • Follows all MCO, Medicare, Medicaid, and HIPAA regulations and requirements.
  • Abides by all regulations, policies, procedures and standards.
  • Answering telephones.

  • Filing/scanning and preparing reports.
  • File reviews.
