About The Position

Join Ambercare/Addus HomeCare as a Service Coordinator to help older adults and individuals with disabilities live safely and independently at home. This role involves scheduling caregivers, ensuring high-quality service, and conducting in-home visits. You will be responsible for managing schedules, supporting field staff, handling client updates, and ensuring care plans are maintained. The position is ideal for someone who thrives in a fast-paced environment and is passionate about making a difference.

Requirements

Must have high school diploma or equivalent.
4 years of office experience.
Interpersonal, organizational, and communication skills.
Computer skills including but not limited to Microsoft Word, Microsoft Excel, and Scheduling program.
Must have reliable transportation and a valid DL for travel to home visits monthly and some marketing.

Responsibilities

Creates work schedules by entering schedules into the system and manages changes due to client request, illness, vacation, or leaves of absence.
Provides alternate coverage to ensure the client's care plan is followed and client services are not interrupted.
Contacts care providers and clients to provide service updates.
Conducts monthly client wellness calls and conducts home visits as required.
Provides thorough, complete follow-through on escalated client complaints and theft claims.
Supervises direct service employees by setting expectations for attendance, performance, and conduct, holding employees accountable to company policies and guidelines.
Ensures the appearance of the branch's open environment is professional: neat, clean, orderly, and generally free of clutter.
Maintains a high degree of confidentiality at all times due to access to sensitive information.
Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the agency.
Follows all MCO, Medicare, Medicaid, and HIPAA regulations and requirements.
Abides by all regulations, policies, procedures, and standards.
