About The Position

The Home Care Scheduler is responsible for the implementation and maintenance of service delivery in the greater Leominster program area. This role serves as a liaison between clients and families, case managers, and Aging Services Access Points (ASAP). The scheduler processes phone referrals, prepares weekly schedules for direct care staff, and monitors service delivery to ensure it is consistent with authorized services. Additionally, the role involves assisting in the evaluation of direct care staff, maintaining office supply inventory, and covering the absence of the Program Administrator.

Requirements

  • Current, valid driver’s license and access to reliable transportation required.
  • Sensitive to the needs of the aged, handicapped, families and children.
  • Knowledge of standard office procedures, practices, and administrative functions.
  • Ability to read, write, and communicate effectively in English.
  • Ability to prepare accurate, professional, and legible reports and documentation.
  • Proficiency in Microsoft Office applications, including Word and Outlook.
  • Ability to exercise sound judgment and make appropriate decisions in a variety of situations.
  • Ability to assess and respond to client needs and effectively implement and follow established care plans.
  • Strong interpersonal and communication skills, with the ability to work effectively with staff, clients, families, and community-based social service agencies.
  • Ability to remain flexible, responsive, and supportive of the needs of others while maintaining reliability and effectively responding to emergency situations.
  • Ability to work effectively and collaboratively in a team environment.
  • Ability to understand and follow written and verbal instructions.
  • Strong organizational skills, initiative, and the ability to recognize and work within the appropriate scope of responsibility.
  • Ability to climb stairs to access all areas of Catholic Charities Worcester County facilities.
  • Normal range of hearing and vision, with or without correction.
  • Ability to sit, stand, walk, bend, lift, reach, and perform routine office and program-related activities.
  • Ability to always maintain confidentiality and professional boundaries.

Nice To Haves

  • Preferred Experience working in a home care setting and familiarity with industry practices.
  • Bi-lingual Spanish/English preferred.

Responsibilities
