About The Position
Touching Hearts at Home is seeking compassionate Registered Nurses (RNs) to support caregivers and clients in the greater Charlotte area and surrounding counties. This role offers a flexible, relationship-centered alternative to hospital settings, focusing on helping older adults live safely and independently at home. It's ideal for nurses who enjoy building relationships, educating caregivers, and improving quality of life without the stress of bedside ratios or demanding schedules. Assignments are territory-based and located close to home.
Requirements
- Active North Carolina RN license
- Current CPR/BLS certification
- Strong communication and interpersonal skills
- Reliable transportation and willingness to travel within assigned territory
Nice To Haves
- Home care, geriatrics, hospice, or community-based nursing experience preferred
- Compassionate, dependable, and relationship-oriented
Responsibilities
- Conduct in-home assessments and develop individualized service plans
- Perform supervisory visits to ensure caregiver performance and client satisfaction
- Train and support caregivers on care plan implementation and best practices
- Monitor client wellbeing and communicate changes or concerns appropriately
- Provide education and guidance to clients and families
- Maintain timely and accurate documentation
- Serve as a trusted resource and advocate for clients and caregivers
