About The Position

BrightStar Care is seeking an RN Case Manager for their growing South Minneapolis Area team. This is a 4-day per week salaried position. The RN Case Manager will work with a team of nurses to complete assessments, re-assessments, and some skilled cares for home care private pay and veteran clients. BrightStar Care is a non-Medicare agency, so no OASIS required. The agency emphasizes a team and family approach to nursing care, aiming to help clients remain safe, happy, and healthy in their homes. The role involves client visits primarily in the Minneapolis Metro Area (Bloomington, Edina, Eden Prairie, etc.). Requirements

• Active RN license in the state of Minnesota.
• Previous experience in a home health or acute care setting is advantageous.
• Exceptional interpersonal and communication skills.
• Committed to delivering high-quality, patient-centered care.

Responsibilities

• Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.
• Collaborate with multidisciplinary team members to optimize patient care and outcomes.
• Educate patients and their families on health maintenance and disease prevention.
• Ensure accurate documentation and compliance with all regulatory guidelines.
