Requirements

  • Current Massachusetts Registered Nurse (RN) license
  • Home care, community health, case management, or visiting nurse experience preferred
  • Strong assessment and clinical judgment skills
  • Excellent communication and documentation skills
  • Ability to work independently
  • Reliable transportation, valid driver's license, and automobile insurance

Responsibilities

  • Complete comprehensive in-home client assessments
  • Develop, review, and update individualized care plans
  • Conduct caregiver orientation and quarterly supervisory visits
  • Educate, coach, and support caregivers to ensure quality care
  • Complete documentation within required timeframes
  • Monitor client progress and communicate changes to the care team
  • Collaborate with office staff to ensure continuity of care
  • Participate in caregiver education and in-service training
  • Perform routine nursing visits as needed, including medication management, blood pressure checks, catheter changes, and other nursing services within your scope of practice
  • Provide hands-on assistance during urgent situations when needed
  • Complete in-home assessments and develop Home Health Aide Plans of Care (for ASAP clients)
  • Match caregiver skills with client care needs (for ASAP clients)
  • Orient and supervise caregivers assigned to ASAP clients
  • Complete quarterly supervisory visits and annual reassessments (for ASAP clients)
  • Collaborate with ASAP nursing staff regarding client status and care plan updates
  • Identify changes in client condition and advocate for appropriate services
  • Report required incidents and changes according to ASAP guidelines
