About The Position

Clover Health is seeking mission-driven physicians to join their field-based In-Home Care team. This role involves caring for Medicare Advantage members with complex medical, functional, and social needs, many of whom are older, medically fragile, and at high risk for avoidable complications. The position focuses on relationship-centered, outcomes-driven medicine, where physicians manage a defined panel of members, understand their home environments, identify risks, and coordinate care. The role offers autonomy, interdisciplinary team support, and technology to enhance care delivery. The schedule is hybrid, with 3 days in the field and 2 days at home (one day virtual health, one day virtual care and admin). Clover is looking for physicians who are highly accountable, clinically strong, operationally effective, and committed to vulnerable seniors. The work directly influences member recovery, stability, hospitalization avoidance, care gap closure, and overall support from a knowledgeable care team.

Requirements

MD or DO degree
Board certification in Internal Medicine, Family Medicine, Emergency Medicine, or a related primary care field
Active New Jersey medical license, DEA registration, and CDS registration
Strong clinical judgment and experience managing chronic conditions in older adults
Comfort caring for medically complex, frail, and socially vulnerable patients
Ability to work independently in a field-based environment
Strong documentation, communication, and care coordination skills
Curiosity and proficiency with clinical technology, including the ability to use tools such as Clover Assistant to make more informed decisions and identify care opportunities

Nice To Haves

Geriatrics experience or fellowship training preferred
Experience in home-based care, value-based care, population health, complex care, or Medicare Advantage is a plus

Responsibilities

Provide high-quality, patient-facing care in members’ homes across Georgia
Manage a panel of medically complex Medicare Advantage members and take full accountability for clinical outcomes
Conduct comprehensive in-home assessments that evaluate medical, functional, behavioral, and social needs
Lead post-discharge and transitional care visits to support safe recovery and reduce avoidable readmissions
Identify changes in clinical status early and intervene proactively
Manage chronic conditions, medication concerns, care gaps, and acute clinical needs within the home-based setting
Build trusted relationships with members, caregivers, and families
Collaborate closely with nurses, care managers, pharmacists, advanced practice clinicians, primary care providers, specialists, and community partners
Use Clover Assistant and other clinical technology tools to surface risks, guide evidence-based decisions, and deliver more proactive care
Communicate clearly and efficiently with internal and external care teams
Document accurately and thoughtfully to support continuity, quality, and accountability
Help advance a care model built around value, outcomes, and human connection
