About The Position

Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Physical Therapist to provide in-home care. This is an opportunity for a Physical Therapist ready to advance their career within a growing organization and a progressive, supportive, and innovative team. All experience levels are welcome, and training will be provided for the right new graduate. The role involves evaluating patients' needs in their homes, developing individualized treatment plans in consultation with physicians, patients, and families, and implementing customized therapy programs to restore function and promote independence. The Physical Therapist will also instruct family members and caregivers on therapy techniques and ensure timely, accurate documentation. Effective communication with patients, families, and the interdisciplinary team, particularly regarding discharge planning, is essential. Care delivery must comply with agency policies and professional practice standards.

Requirements

  • Physical Therapist Program Graduate
  • Licensed Physical Therapist in the state of New York or eligible
  • Ability to travel to patients’ homes within the assigned geographic territory

Nice To Haves

  • Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.

Responsibilities

  • Accept and treat patients within your designated geographic area
  • Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
  • Develop a plan of care and treatment goals in consultation with the physician, patient, and family
  • Plan, prepare and implement customized therapy programs to restore function and promote independence
  • Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
  • Provide timely and accurate documentation for each visit
  • Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
  • Deliver care in compliance with agency policies and professional practice standards
