About The Position

Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Physical Therapist to provide in-home care. This is an opportunity for a Physical Therapist to advance their career within a growing organization that fosters a progressive, supportive, and innovative team environment. New graduates are welcome, and training will be provided. The role involves treating patients within a designated geographic area, evaluating their needs, and developing individualized treatment plans. The therapist will also instruct family members and caregivers on therapy techniques and ensure timely, accurate documentation.

Requirements

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New York or eligible
• Ability to travel to patients’ homes within the assigned geographic territory

Nice To Haves

• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.

Responsibilities

• Accept and treat patients within your designated geographic area
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family
• Plan, prepare and implement customized therapy programs to restore function and promote independence
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards
