About The Position

The Home Care MSW Social Worker at EHM Senior Solutions is responsible for providing comprehensive social work services to clients in their homes. This role involves assessing the psychosocial needs of clients, developing care plans, and coordinating with healthcare providers to ensure the delivery of quality care. The social worker will support clients and their families in navigating healthcare systems, accessing community resources, and improving their quality of life.

Requirements

  • Master’s degree in Social Work (MSW) from an accredited institution.
  • Must hold an active MSW in the State of Michigan.
  • Minimum of one year of experience in a social work role, preferably in a home care setting.
  • Experience working with elderly populations and understanding their unique needs.
  • Proven track record in case management and developing care plans for individuals with varying health conditions.
  • Strong interpersonal and communication skills to effectively interact with clients, families, and healthcare professionals.
  • Ability to assess client needs and provide appropriate resources and support.
  • Excellent organizational skills and the ability to manage multiple cases simultaneously.
  • Proficiency in using electronic health records and other relevant software.
  • Strong problem-solving skills and the ability to work independently as well as part of a team.
  • Understanding of local community resources and the ability to connect clients with necessary services.
  • Compassionate and empathetic approach to client care, with a commitment to improving their quality of life.

Responsibilities

  • Conduct thorough assessments of clients' psychosocial needs and develop individualized care plans.
  • Provide counseling and support to clients and their families to address emotional, social, and financial challenges.
  • Collaborate with healthcare providers, including nurses and physicians, to coordinate care and ensure a holistic approach to client health and well-being.
  • Assist clients in accessing community resources and services, such as financial assistance, transportation, and support groups.
  • Advocate for clients' rights and needs within the healthcare system and community.
  • Maintain accurate and up-to-date documentation of client interactions, assessments, and care plans.
  • Participate in interdisciplinary team meetings to review and update client care plans.
  • Stay informed about changes in healthcare regulations and community resources to provide the most current and effective support to clients.
