About The Position

Lehigh Valley Health Network (LVHN) is seeking a Medical Social Worker (MSW) for their Home Care and Hospice team. This role involves providing psychosocial assessments, counseling, crisis intervention, and information/referral services. The MSW will collaborate with the management team to ensure efficient resource utilization for positive patient outcomes. Requirements

Master’s Degree in Social Work
1 year experience working as a social worker or applicable mental health experience.
Ability to work effectively with people from a broad range of backgrounds.
Ability to adhere to the National Association of Social Workers Code of Ethics.
Knowledge of patient rights and laws relative to those rights, such as HIPAA.
Knowledge of psychological principles and theories.
Must possess a high degree of emotional intelligence, and empathy towards others.
Skill in using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems.

Responsibilities

Provides psychosocial assessment, counseling, crisis intervention, and information/referral services on a consultation basis.
Works collaboratively with the management team to ensure the efficient utilization of resources for positive patient outcomes.
Employs counseling skills to help patients/families understand, accept, and follow recommendations for care and to restore patients to optimum social and health adjustment.
Stays up-to-date on current trends and researches the availability of community agencies and community resources for social, emotional, and financial assistance.
Intervenes in crisis situations and effectively applies crisis intervention theory techniques.
Identifies and records clinical, psychosocial, and financial barriers to a smooth transition across the healthcare network.
Participates in a monthly or yearly audit or review to ensure adherence to laws and other federal guidelines.
Works collaboratively with the patients and patient's social support system to establish and implement a plan.
Interviews the patient to complete the initial psychosocial evaluation.
Reports suspected cases of child and adult abuse/neglect/exploitation.
