About The Position

The Home Care Services Manager is responsible for operational over-site of the PACECare at Home nursing assistants. In addition, the Home Care Manager is responsible for facilitating specified Interdisciplinary Team meetings. Requirements

Bachelor required
Must meet a standardized set of competencies (approval by CMS) before working independently.
Must be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
Must meet State Licensure requirements (be legally authorized to practice in Michigan, and only act within the scope of his/her authority to practice).
Excellent oral and written communication skills necessary; must have strong interpersonal skills.

Nice To Haves

Bachelor of Science in Nursing preferred.
Preferred three years of clinical long-term care experience, including one year with frail elderly population, and two years of supervisory experience.
Has ability to lead and direct other licensed staff in the delivery of care.
Only acts within scope of his/her practice.

Responsibilities

Supervises and coordinates Home Care Activities at PACE Southeast Michigan (PACE SEMI)
In collaboration with the Center Manager and IDT, facilitates the Participant-Centered Planning meetings, as well as complex Care Plan Development/Enrollment meetings, to assure consistency in care planning and service authorization for participants enrolled at PACE SEMI.
Assists the Medical Director as requested in duties related to clinical and operational issues and special projects as may be required.
Provides home care authorizations for external vendors as needed based on staffing to ensure participant needs are met
Teach, coach and mentor schedulers along with Direct Care staff.
Communicates with the Director of Home Care regarding case management/homecare operations and issues.
Facilitates administrative, quality and performance improvement requirements, such as reports and statistical records.
Provides weekly report to schedulers regarding staff productivity.
Participates in and/or facilitates Quality Assurance projects resulting from data results.
Ensures adherence to departmental and external standards in the provision of quality focused care by attendance at professional meetings/committees and review of national standards of practice.
Ensures adherence to budgetary guidelines.
Other duties as assigned.
