About The Position

The Home Care Services Manager is responsible for operational over-site of the PACECare at Home nursing assistants. In addition, the Home Care Manager is responsible for facilitating specified Interdisciplinary Team meetings. Requirements

  • Bachelor required
  • Must meet a standardized set of competencies (approval by CMS) before working independently.
  • Must be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
  • Must meet State Licensure requirements (be legally authorized to practice in Michigan, and only act within the scope of his/her authority to practice).
  • Excellent oral and written communication skills necessary; must have strong interpersonal skills.

Nice To Haves

  • Bachelor of Science in Nursing preferred.
  • Preferred three years of clinical long-term care experience, including one year with frail elderly population, and two years of supervisory experience.
  • Has ability to lead and direct other licensed staff in the delivery of care.
  • Only acts within scope of his/her practice.

Responsibilities

  • Supervises and coordinates Home Care Activities at PACE Southeast Michigan (PACE SEMI)
  • In collaboration with the Center Manager and IDT, facilitates the Participant-Centered Planning meetings, as well as complex Care Plan Development/Enrollment meetings, to assure consistency in care planning and service authorization for participants enrolled at PACE SEMI.
  • Assists the Medical Director as requested in duties related to clinical and operational issues and special projects as may be required.
  • Provides home care authorizations for external vendors as needed based on staffing to ensure participant needs are met
  • Teach, coach and mentor schedulers along with Direct Care staff.
  • Communicates with the Director of Home Care regarding case management/homecare operations and issues.
  • Facilitates administrative, quality and performance improvement requirements, such as reports and statistical records.
  • Provides weekly report to schedulers regarding staff productivity.
  • Participates in and/or facilitates Quality Assurance projects resulting from data results.
  • Ensures adherence to departmental and external standards in the provision of quality focused care by attendance at professional meetings/committees and review of national standards of practice.
  • Ensures adherence to budgetary guidelines.
  • Other duties as assigned.
