About The Position

St. Mary's Home Care & Hospice is seeking a Full Time LPN for Homecare revisits. This role involves providing one-to-one, compassionate care to patients in their homes. Our home care LPNs utilize cutting-edge technology and clinical knowledge to offer exceptional care, focusing on client assessment, care planning, intervention, and supervision of patient care and outcomes. Nurses follow physician orders and adhere to the state's Nurse Practice Act and company policies. The position is based in Athens, GA, and is part of an integrated virtual care program designed to enhance clinical excellence and help patients avoid preventable ER visits and hospitalizations.

Requirements

Graduate of an accredited or state-approved school of practical nursing.
Licensure as an LPN in the State of Georgia.
A minimum of one year of experience in an acute care setting.
Phlebotomy and wound care experience required.
Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.

Nice To Haves

Home care experience preferred.

Responsibilities

Provide one-to-one care with patients in their homes.
