About The Position
MySolutions Home Care, part of Lutheran Life Communities, is hiring a Home Care Coordinator in Arlington Heights. This role involves managing the intake and care coordination for clients receiving MySolutions Home Care services. The coordinator will help residents and families navigate care transitions smoothly, from initial intake through ongoing home visits.
Requirements
- High school diploma or equivalent
- Valid driver's license, for regular home visits across the service area
- Able to read, write, and comprehend English instructions and documentation
- Basic math and computer proficiency

- Background in healthcare, social work, sales, or senior living
- Supervisory experience
Responsibilities
- Manage intake calls and follow up on incoming leads for MySolutions Home Care services
- Visit new residents in MyRehab shortly after admission and again before discharge to support smooth care transitions
- Assess client needs and develop individualized service plans
- Oversee delivery of home care and handyman services
- Conduct regular home visits, 90-day check-ins, annual care plan reviews, and ongoing assessments
- Build strong relationships with clients, families, and caregivers, and connect them with community resources
- Assist with caregiver scheduling, onboarding, and training
