About The Position

Gary and Mary West PACE is a non-profit organization affiliated with West Health, dedicated to enabling seniors to age in place independently by providing comprehensive health and supportive care. They focus on delivering medical, social, and behavioral services to the frail elderly in North County San Diego, with an entrepreneurial approach to innovation within the traditional PACE model. Requirements

  • High school diploma/GED required.
  • Current driver license.
  • Proof of auto insurance.
  • Clean driving record.
  • BLS and CPR Certification.
  • Minimum of two (2) years of demonstrated successful experience in home care; prefer in-home care management experience.
  • Minimum of one (1) year of documented experience working with a frail or elderly population.
  • Experience in the provision of person-centered care, including assessment skills, personal care, and health education.
  • Working knowledge of physical, mental, spiritual, and social needs of the frail elderly and their families.
  • Effective time management skills.
  • Knowledge of utilization review, quality improvement and cost containment systems.
  • Ability to work with ethnically diverse populations.
  • Ability to apply creative problem-solving skills to the complex issues in home care.
  • Ability to work with interdisciplinary teams.
  • Strong organizational skills, flexible, resourceful, and energetic.
  • Effective oral and written communication skills.
  • Medical clearance for communicable diseases and up-to-date immunizations before direct participant contact.
  • Successful pre-employment drug screen, background check, and physical assessment.

Nice To Haves

  • LVN/LPN preferred.

Responsibilities

  • Planning, monitoring, and scheduling of in-home services.
  • Working closely with the Personal Care Supervisor (PCS) to ensure all personal care needs of participants are met.
  • Ensuring home care schedules are maintained, accurate, and participants are allocated sufficient time/services.
  • Coordinating care through staffing with contracted home care providers and Personal Care Attendants (internal staff).
  • Coordinating visits, considering the qualifications of the Personal Care Attendant and the participants' level of care.
  • Re-assigning daily visits if a staff member cannot conduct visits as scheduled and informing the PCS when visits cannot be rescheduled.
  • Proactively modifying the schedule to ensure accuracy and notifying participants/caregivers in advance of changes.
  • Participating as a member of the Interdisciplinary Team (IDT) in assessments (initial, semi-annual, unscheduled, and annual).
  • Attending morning updates and reporting changes in participants' status to appropriate staff.
  • Performing homecare assessments for Service Delivery Requests (SDR) and updating the EMR per regulatory guidelines.
  • Collaborating with the PCS and IDT, potentially participating in initial enrollment meetings.
  • Submitting authorization requests to home care agencies as required.
  • Filling in as a PCA when the need arises or in emergent situations.
  • Assuring that documentation for home care is accurate, current, and entered in the EMR timely, in conjunction with the PCS.
  • Maintaining confidentiality regarding participant, staff, contractor, and organizational information.
  • Attending and participating in staff meetings, in-services, projects, and committees as assigned.
  • Adhering to and supporting the center's policies, practices, and procedures.
  • Accepting assigned duties in a cooperative manner and performing all other related duties as assigned.
  • Being flexible in schedule of hours worked.
  • Using a personal vehicle for work-related travel.
