About The Position

This position provides personal care and/or home management tasks to clients requesting and eligible for Caregiver services, under the direction of the RN. The role involves assisting clients with daily living activities, maintaining their living environment, and communicating any changes in client condition to the registered nurse. Documentation of care provided and adherence to safety and infection control protocols are essential. The position requires the ability to work flexible hours and coordinate with various departments.

Requirements

No CNA License Required
Valid driver’s license and proof of automobile insurance
Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneeling
Flexion/Extension of arms/wrist
Hand dexterity to include gripping, pushing/pulling and writing
If assigned to hands-on client, ability to assist with transfers/repositioning
If assigned to hands-on client, lifting/carrying of 50 pounds
Knowledge and application of proper body mechanics
Knowledge and application of Infection Control Principles & environmental safety
Excellent organization skills
Excellent interpersonal skills
Excellent written and oral communication skills
Ability to work under pressure to meet deadlines
Ability to take initiative and utilize innovative techniques and ingenuity

Responsibilities

Practice within the scope of training, license, certification, or registration, where applicable, and as required by law and regulation.
Perform tasks as delegated and directed by the registered nurse.
Assist clients with Activities of Daily Living and/or Instrumental Activities of Daily Living, including but not limited to: Assisting with ambulation, transfers, or exercises, Assisting with bathing, dressing, and grooming, Assisting with feeding, Assisting with toileting, and incontinence care, Housekeeping, Meal preparation, Safe transportation, Medication reminders, Pet care, Maintain general order, safety, and cleanliness of Client area.
Communicate any changes in Client condition or occurrences to registered nurse.
Maintain chart documentation of care provided in accordance with the written plan of care.
Practice safety and infection control methods.
Demonstrate proper transfer and lift techniques using good body mechanic techniques.
Maintain confidentiality & privacy of any PHI per HIPAA requirements.
Exhibit competency in the observance of Universal Precautions by: Knowing standard precautions, Washing hands before and after patient care, Wearing gloves and other PPE as required.
Reports to work on time, ready for shift, and appropriately dressed for role.
Consistently demonstrates Four Seasons values and competencies.
Completes required orientation and on-going training.
Participates in ongoing education and training offered by the agency and/or outside resources as required for role.
Practices professional boundaries and behaviors and communication.
Practices personal and environmental safety.
Perform other duties as assigned by registered nurse, office manager, or director.
