About The Position

When a family calls MySolutions, they are usually worried about a parent or spouse. As our Home Care Client Coordinator, you are the person who calls them back, meets them, and builds a care plan that works. Then you stay in touch to make sure it keeps working. MySolutions is the licensed home care service of Lutheran Life Communities, a not-for-profit that has served older adults for more than 130 years. We welcome team members of all faiths and backgrounds.

Requirements

Valid driver's license and reliable transportation for home visits
Ability to pass the background check required for Illinois home care agencies

Nice To Haves

Experience in sales, healthcare, social services, or therapy
Comfortable talking with families during stressful moments
Comfortable with phone calls, video meetings, and basic computer work

Responsibilities

Reach out to every new lead within 24 hours
Meet clients and families by phone, video, or home visit to learn what they need
Explain our home care services and help families choose the right fit
Build a care plan and secure the signed service agreement
Share care plan details, staffing needs, and client preferences with our Program Manager so care starts smoothly
Keep client records complete and on time
Check in with families and solve problems so clients stay happy and well cared for
