About The Position

LifePath is a mission-driven, collaborative, and community-focused human services agency seeking a Home Care Case Manager to join our caring and compassionate team! The Case Manager is responsible for developing supportive relationships with older adults, fostering growth in independence, and serving as a central source of support and resources. This position is also responsible for the evaluation, needs assessment, and overall implementation of Home Care services for eligible applicants. Responsibilities include dissemination of information and making appropriate referrals, setting up and monitoring a service plan for Home Care services for each client, and serving as an advocate for clients. This is a full-time (35 hr/wk) role based out of our Greenfield office, serving Franklin County and the North Quabbin area. Pay rate $22.25/hour with excellent benefits including generous PTO, medical/dental/vision insurance, Flex spending, EAP, 401k and more! 6% pay differential for applicable bilingual skills.

Requirements

  • Bachelor’s Degree preferred and 2-3 years of previous experience in health or human services required.
  • Willingness and sensitivity for working with older adults.
  • Strong documentation skills and familiarity with electronic health records (EHR) or care management platforms and Microsoft programs.
  • Ability to work independently and as part of an interdisciplinary team.
  • Exceptional interpersonal and communication skills
  • Capacity to complete comprehensive needs assessments using observation, listening, and interviewing skills.
  • Ability to solve problems and find solutions.
  • Flexible and able to meet multiple,and changing,program demands.
  • Familiarity with or willingness to learn about health and human services programs.
  • Demonstrates cultural humility and cultural responsiveness.

Nice To Haves

  • License in Social Work is a plus.

Responsibilities

  • Conduct in-person comprehensive, person-centeredassessmentswithconsumers, care partners, andhealth-care professionals.
  • Develop, implement, and updatea service plan with the consumerin collaboration with their formal and informal support systems
  • Authorize internally funded services.
  • Make appropriate referrals to other community-based agencies and governmental organizations
  • Collaborate with internal and external providers to ensure adequate coordination of service plans (to include meetings and telephone contacts).
  • Adhere to documentation standards and maintenance of consumer recordsaccording to regulations.
  • Assistclients, families,caregivers,and informalsupportsin problem-solving. Facilitate team meetings and case/family conferences as needed.
  • Maintain up-to-date knowledge of information about services available toolder adults intheservicearea.
  • Serve as an advocate for consumers to obtain other needed services.
  • Participate in interdisciplinary team meetings as needed.
  • Act as a representative of LifePath that times when directed to do so.
  • Provide Office Coverage as assigned.
  • Participate in staff meetings and trainings as required.
  • Assisting training new staff by allowing them to shadow visits and by sharing information and tips.
  • Otherduties as assigned by immediate supervisor, otherdesignatedsupervisorystaff,ordesignatedmember oftheManagementTeam.
