About The Position

Always Best Care of Greater Lexington is looking for a motivated, experienced self starter to fill the position of Care Coordinator. This is a leadership position within our Care Team that has endless possibilities for advancement for the person with the right combination of skills, attitude, reliability, and dedication to quality of work and client care. The primary job duty is covering client shifts when the regular caregiver is out and covering new starts of care with excellence and enthusiasm. Additional job duties include facilitating Care Team training and development, assisting with admin on call duties on a rotation, Care Team mentorship, Client Quality Assurance, and various office support duties as assigned when not covering clients. Frequent driving within Lexington/Fayette and surrounding counties is required, so a car in good condition is essential.

Requirements

  • Must be at least 21 years of age
  • Able to pass a drug screen, criminal background check, DMV check and a check of the Nurse Aide Abuse Registry and Caregiver Misconduct Registry
  • Willing and physically able to perform all aspects of client care including assisted transfers and bed bound care
  • Able to lift and carry 25 lbs a distance of 25 ft several times a day
  • Able to read instructions
  • Spend large amounts of the day on your feet
  • Reliable vehicle
  • Valid driver's license
  • Car insurance
  • Fluent in the use of smart phones
  • Basic computer skills
  • A positive, outgoing attitude is a MUST!

Nice To Haves

  • SRNA/CNA or working towards certification
  • Two years experience in a Skilled Nursing or Hospital environment performing hands on care
  • Home Health and In Home Care experience also considered
