Home Care Assistant / Certified Nursing Assistant (CNA)

Requirements
- High school diploma or equivalent.
- Current North Carolina Certified Nursing Assistant (CNA) certification.
- Current CPR certification.
- Minimum of one year of experience working with older adults, seniors, or frail populations.
- Valid driver's license, reliable transportation, and proof of insurance.
- Ability to lift up to 40 pounds and perform the physical demands associated with direct care.

Responsibilities
- Provide personal care and assistance with activities of daily living, including bathing, dressing, grooming, oral care, toileting, and meal support.
- Deliver home care services in accordance with individualized care plans and physician-directed services.
- Observe and report changes in participants' physical, emotional, or behavioral status to the clinical team.
- Assist with restorative and supportive care activities that promote independence and well-being.
- Facilitate and participate in group and individual activities designed to enhance engagement and quality of life.
- Support participants during meals and encourage proper nutrition and hydration.
- Accompany participants to medical and community appointments when needed.
- Assist with transportation activities, including participant loading, unloading, and escorting.
- Maintain accurate documentation and participant records.
- Contribute to a safe, clean, and welcoming environment.
- Collaborate with nurses, therapists, caregivers, and other interdisciplinary team members to deliver exceptional participant-centered care.
