About The Position

Sea Mar Community Health Centers is seeking a Home Care Aide to provide non-medical assistance to clients in their homes. The role involves performing basic household management and personal care tasks, following a client's care plan. The goal is to help clients remain in their homes and delay institutionalization. This position requires adherence to specific protocols, including client identification, medication reminders, and reporting any skin issues or changes in client condition to a supervisor. The Home Care Aide will also be responsible for maintaining a safe and healthy living environment for the client.

Requirements

  • A minimum of one year of experience in household management, paid or unpaid.
  • Must pass a Department of Social & Health Services background check.
  • Ability to use computer, mouse and keyboard for training purposes.
  • Ability to use a “smart” phone (Apple iPhone or Android) for calls, downloading apps, entering data, and obtaining client signatures.
  • Ability to add, subtract, multiply, and divide.
  • Ability to respond to emergent client behavior and/or rapid changes of client medical stability.
  • Must obtain DSHS HCA certification in no more than 200 calendar days.
  • Must have and maintain a current TB test.
  • Must be current with standard health immunizations.
  • Must provide evidence of COVID 19 Vaccinations and Booster.
  • Must provide evidence of receiving an annual flu vaccine.
  • Must provide a copy and maintain a current Washington State Driver’s License.
  • Must provide proof and maintain current automobile insurance.
  • Must have a clean driving record when assigned to transport clients or run errands.
  • Must successfully complete 12 (twelve) hours of annual Continuing Education prior to his/her birthdate.

Nice To Haves

  • Prior experience with adults unable to care for themselves.
  • Demonstrates knowledge and skills necessary to provide care appropriate to the age of the clients served.
  • Bilingual desired (Spanish preferred).
  • High school diploma or GED preferred.
  • Previous experience in a nursing home or geriatric setting is preferred.

Responsibilities

  • Perform personal care tasks including assistance with toileting, bathing, dressing, and transfers.
  • Provide medication reminders, ensuring client identifiers and medication expiration dates are checked.
  • Monitor and report any skin redness, discoloration, or other skin issues to the home care supervisor.
  • Assist with ambulation, including providing support or pushing wheelchairs.
  • Measure pain using the Wong-Baker Faces Pain Rating Scale and record the level of pain.
  • Provide escort/transportation for medical appointments, essential shopping, and errands.
  • Ensure the safety of clients who are unable to recognize or respond to unsafe situations.
  • Perform household management tasks including housekeeping (sweeping, mopping, dusting, trash removal, changing linens, cleaning kitchen/bathroom), laundry, and meal preparation.
  • Occasionally perform snow removal and wood chopping if it's the client's sole source of heat/cooking.
  • Maintain a safe and healthy living environment for assigned client(s).
  • Clock in and out appropriately in the Matrixcare Mobile App and ensure client signature.
  • Notify a Supervisor of absences or any need to change a client’s schedule.
  • Attend all scheduled meetings and required training.
  • Observe and report all client or service-related problems to a Home Care Supervisor.
  • Report all accidents to a Home Care Supervisor.
  • Be available to work the agreed-upon hours and days.
  • Perform other duties as assigned.
