The assessor will build rapport with participants and their families/caregivers, administer structured assessments using electronic systems, and document information about daily living skills, behaviors, and support needs. The goal is to ensure individuals maintain access to community-based services that support their independence and quality of life. This position requires residing in Southeast New Mexico and access to reliable personal transportation for regional travel.
