About The Position

Under the direction of the ID Program Manager, the HIV Prevention Navigator provides comprehensive, patient-centered navigation for individuals eligible for or receiving HIV Pre-Exposure Prophylaxis (PrEP). The role includes tracking injectable medication schedules; helping patients use MyChart and telehealth; maintaining accurate EHR documentation; supporting Ryan White grant compliance and data collection; and participating in community outreach, health equity initiatives, team activities, and quality improvement. The position serves populations disproportionately affected by HIV, including cis women and cis women of color, and requires culturally responsive communication, confidentiality, and collaboration across multidisciplinary teams.
