The Case Manager I is responsible for various tasks facilitating the client’s ability to engage in and remain in medical care. The Case Manager's specific tasks vary depending on the program to which the staff member is assigned. The Case Manager's duties include patient advocacy, applying for public benefits, completing applications for the AIDS Drug Assistance Program (ADAP), determining Ryan White eligibility, providing outreach to clients at risk of or who have fallen out of care, scheduling transportation, and completing monthly reports and special projects. The Case Manager I works as an integral part of the Psychosocial Program staff.
