About The Position

Ward 86, the HIV/AIDS Clinic at the Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG) campus, is a leading innovator in HIV care. As the largest clinic providing primary medical care for people living with HIV for the San Francisco Health Network (SFHN), Ward 86 also offers preventive services like pre-exposure prophylaxis (PrEP) to at-risk patients. The clinic serves approximately 2500 patients living with HIV, with over 24,000 clinical encounters annually. Care is delivered through a comprehensive, multi-disciplinary approach, encompassing primary medical and HIV care, urgent care, mental health services, substance use assessment and referrals, HIV prevention, specialty HIV care (Pulmonary, Dermatology, Cardiology), and psycho-social assessment and interventions.

Requirements

  • Experience providing supportive linkage to health care services.
  • Experience rendering dedicated and ongoing assistance to patients to stay engaged in HIV care.
  • Experience assisting patients with linkage to care.
  • Experience providing engagement interventions for patients who are at risk of falling out of care.
  • Experience with navigation of patients to care.
  • Experience providing adherence counseling.
  • Experience conducting follow-up outreach in person, by phone, or other electronic means.
  • Experience liaising with clinical service providers to address specific health concerns.
  • Experience assisting with the development, modification, and implementation of HIV care and preventative protocols.
  • Experience reviewing patient charts.
  • Experience entering/tracking patient data via electronic health records and other data systems.
  • Ability to perform work at the ZSFG Campus and in the community within the City and County of San Francisco.

Responsibilities
