About The Position

Unity House of Troy, Inc., a non-profit multi services agency dedicated to enhancing the lives of people in need has an opening for a Social Determinants of Health Liaison in our Supported Housing program for those living with HIV/AIDS. This position has the option of a hybrid schedule. Unity House is a Rensselaer County-based human service agency that provides a wide range of services to meet the otherwise unmet needs of people in our community who are hurting and struggling. We assist those who are living in poverty, adults living with mental illness or HIV/AIDS, victims of domestic violence, and children with developmental delays. We work to achieve social justice in our community and to create a better understanding of those we serve.

Requirements

  • Associate’s or Bachelor’s Degree in health, human, or educational services or equivalent preferred. Experience may be substituted for degree.
  • At least one-year experience working with those living with HIV/AIDS or other at-risk populations.
  • Experience working with adults living with mental illness and/or substance abuse issues a plus!
  • High School diploma is required.
  • You must have a Valid and clean New York State Driver License and access to a reliable vehicle.
  • Strong interpersonal, organizational skills, and experience/comfortability with computer systems and data entry required.

Responsibilities

  • Refer and link clients to supportive services that improve medical outcomes.
  • Assist participants to access emergency financial assistance, transitional housing and other resources.
  • Complete assessments, support plans, and other required program documentation.
  • Improve access to care though referral, linkages and care coordination.
  • Collaborate and act as a liaison with other services providers in the community.
  • Obtain, maintain, enter and track data for reports related to funding compliance.
  • Promote a culture of inclusion and belonging.
  • Practice a harm-reduction service philosophy.
