This position is responsible for helping the Medical Staff and organization meet Federal and State compliance performance standards through accurate audits and timely report of incomplete encounters. This position is also responsible for the release of information function in accordance with Federal and State laws, as well as hospital policy and procedure, admission of patients, birth certificate completion, maintaining knowledge and understanding of duties related to the processing of outpatient records, filing, retrieval, archiving, and computer input. This is a full-time hybrid position working Mon- Fri from 7:00am-3:30pm. Approximately 50% remote with On-Site/Hospital coverage requirements. WORK ENVIRONMENT The professional in this position reports to the Manager of Health Information Management. This position works closely with other healthcare professionals in order to provide high quality services to all customers.

Requirements

  • High school diploma or equivalent required.
  • 1 or 2 years of hospital or medical office release of information experience.
  • Must be able to operate personal computer, calculator, fax machine, microfiche or microfilm reader/printer, scanner, and photocopier.
  • Demonstrates competency on equipment listed on department specific checklist.
  • Critical thinking skills: Seeks resources for direction, when necessary. Performs independent problem solving. Decision-making is logical and deliberate. Performs actions that demonstrate accountability. Exercises safe judgment in decision-making. Practices within legal and ethical guidelines.

  • Occasional standing, walking, lifting, reaching, kneeling, bending, stooping, pushing and pulling.
  • Light physical effort, ability to lift up to 10 lbs.
  • Ability to communicate using verbal and/or written skills for accurate exchange of information with physicians, nurses, health care professionals, patients and/or family, and the public.

Responsibilities

  • Maintains confidentiality of the medical records, reports and hospital business.
  • Performs chart review and maintains accurate records of incomplete chart encounters by following procedures and Medical Staff policy.
  • Submits required reports to the Director and Medical Staff Office in a timely manner.
  • Provides connection between HIM and Medical Staff Office for Medical Staff Peer Review process.
  • Generates monthly and quarterly reports on data collected and submits reports to the Director and appropriate hospital committees.
  • Handles MPI reassignment list/ general corrections for patient admissions at HIM level.
  • Acts proficiently and courteously.
  • Receives, files, retrieves, analyzes and routes outpatient and ancillary records within the Samaritan Hospital system.
  • Notifies Outpatient Department of missing records.
  • Maintains professional growth and development through seminars, workshops and professional affiliations to keep abreast of latest trends in field of expertise.
  • Prepares records for Coders.
  • Demonstrates knowledge of the rules and regulations regarding legal procedures.
  • Retrieves medical records for continued patient care, committee meetings, Quality Improvement activities, QIO, etc, if needed.
  • Demonstrates knowledge and proficiency in maintenance of files and indices.
  • Performs general office and clerical duties (i.e. answer phones, distribute mail and maintain general office supplies).
  • Operates office equipment.
  • Locates and transports charts to E.R. and nursing stations as needed.
  • Knowledge of State rules and regulations regarding birth certificate completions.
  • Knowledge of the rules and regulations regarding legal procedures for the release of medical information and patient privacy.
  • Records accurate information in the birth log and keeps monthly birth statistics.
  • Maintains fetal monitor strips.
  • Knowledge and proficiency in maintenance of files and indices.
  • Performs archiving duties as required.
  • Performs notary functions.
  • Prepares Paternity Affidavits/Support Enforcement forms for state offices.
  • Maintains professional growth and development through seminars, workshops and professional affiliations to keep abreast of latest trends in field of expertise.
  • Ensures no injuries to self or others by following safe work practices and policies. This includes, but is not limited to: security and safety, understanding of MSDS, equipment, infection control, fire, disaster, safe lifting and body mechanics.
  • Ensures self-compliance with organization policies and procedures as well as labor agreements.
  • Ensures the interface with team members and other support groups is conducted in a courteous and efficient manner conducive with the organization’s values.
  • Conducts self in a professional manner and ensures personal appearance meets the standards necessary to perform the job function while representing the organization.
  • Ensures that additional accountabilities, as may be required by management, be handled in a manner necessary to meet organizational standards.
