This is an onsite position at Ascension St. Vincent Kokomo. As our HIM Associate II, you will help maintain the accuracy, completeness, and integrity of the legal health record to support quality patient care, regulatory compliance, and timely reimbursement. Every day you will prepare, scan, and index medical record documentation, work DNFB accounts and monitor completion of discharge summaries and operative reports, and review data integrity task queues to support EMPI functions and chart corrections. To thrive in this role, you must be highly organized, detail-oriented, professional, and able to manage multiple HIM responsibilities while protecting patient confidentiality.

Requirements

  • High School diploma or equivalent required
  • Strong communication, organization, time management, professionalism, and confidentiality skills.
  • Ability to operate computers, telephones, and office equipment, including frequent use of hands and fingers for typing and document handling.
  • Ability to utilize office equipment, perform detailed document tasks, and occasionally transport records or supplies, including pushing/pulling carts and lifting over 5 lbs.

Responsibilities

  • Prepare, scan, and index medical record documentation to ensure the legal medical record contains all required information related to patient care, regulatory compliance, legal standards, and reimbursement activities.
  • Work the Discharged Not Final Billed (DNFB) list to resolution.
  • Monitor the timely completion and entry of discharge summaries and operative reports.
  • Review and manage task queues for data integrity issues, including duplicate, overlapping, and overlaid records; collaborate with the EMPI team to ensure timely chart corrections.
  • Perform quantitative analysis of medical records and monitor the quality and accuracy of work performed by global team members.
  • Maintain required data collection logs and spreadsheets.
  • Perform additional departmental duties as assigned.
