About The Position

Youth Villages is seeking purpose-driven individuals passionate about making a difference in the lives of children and families. The High Fidelity Wraparound (HFW) model is an evidence-based, team-driven care management approach designed to support youth and families with complex behavioral and health needs. Serving youth ages 3 to 17, HFW provides coordinated, strengths-based support that empowers families to achieve long-term stability, resilience, and success in their homes and communities. The program ensures that services are youth-guided and family-driven, with families collaborating with providers and natural supports to form a personalized team. Together, they develop and monitor a plan of care tailored to the family’s unique needs and goals, which can include building coping and problem-solving skills, creating safety plans, strengthening family relationships, improving school engagement, and enhancing formal and natural support networks.

Requirements

• Must possess degree at time of application.
• Degree must be in a clinical field of study.
• Must have two years of full-time clinical experience (4,000 hours) with designated population (for Bachelor's Degree applicants).
• Must have one year of full-time clinical experience (2,000 hours) with designated population (for Master's Degree applicants).
• Must hold one of the following licenses in the state of North Carolina: LCMHC-A, LMFT-A, LCSW-A, or LCAS-A (for Clinical Licensed applicants).
• Strong organizational skills and attention to detail
• Excellent written, verbal, and oral skills
• Ability to manage multiple priorities simultaneously
• Basic computer knowledge
• Ability to maintain a flexible schedule
• Must possess a current, valid driver’s license, an automobile for work purposes, and proof of auto insurance.

Responsibilities

• Carries small caseload of 10 to 12 families
• Facilitates High Fidelity Wraparound services including utilizing a strengths-based approach to safety planning, development of family team and family support systems, and wraparound planning for the purpose of maintaining children in their homes, schools, and communities
• Prepares Wraparound Plan with families and agency partners with measurable outcomes
• Provides on-call crisis support to the youth and family (schedules vary by location)
• Attends 3 weekly meetings (individual, team, and clinical consultation) for professional development and quarterly booster training sessions to enhance clinical skills
• Completes accurate and timely documentation in an electronic medical record system (EMR)
• Performs other duties as assigned
