About The Position

High-Fidelity Wraparound is an evidence and team-based, person-centered process which provides coordinated, holistic, family-driven care to meet the complex needs of children/youth who are involved with multiple systems. The overall goal is to support youth and their families in achieving and maintaining desired mental and behavioral health outcomes at home, in the community, and in the least restrictive environment possible. Wraparound Facilitators engage families to identify needs and teach them skills in navigating systems and accessing resources to address those needs. Facilitators develop and lead a Child and Family Team (CFT) comprised of both formal and natural support persons who assist the youth, and family in prioritizing needs and developing a Wraparound Plan which includes Crisis Stabilization and/or Prevention Plans. Facilitators convene CFT Meetings, coordinate and communicate with the participants and members of the team to ensure the implementation of the Wraparound Plan, coordinate the delivery of available supports and services, monitor and review progress toward goals and update the Wraparound Plan as necessary.

Requirements

  • Successful completion of a bachelor’s degree in a human behavioral science field or other appropriately related field.
  • Minimum of two (2) years of full-time professional experience working in human services or a behavioral healthcare setting. OR Successful completion of a master’s degree in a human behavioral science field and one (1) year of professional experience working in human services or a behavioral healthcare setting.
  • Experience working with children and families who have experienced trauma with an understanding of how trauma can impact mental/behavioral health.
  • Experience working with individuals with behavioral/mental health challenges.
  • Must possess the ability to keyboard at 40 words per minute.
  • Must provide own transportation and possess a valid Colorado Driver's License and Liability Insurance.
  • Must pass Human Services Background Checks. Background checks include CBI/FBI fingerprinting and will continue to be monitored throughout employment.
  • Must be able to work evenings and weekends when required.
  • Must have knowledge of computer programs such as Microsoft Windows and Word, Excel, and Outlook.

Nice To Haves

  • Experience working within Child or Adult Protection.
  • Experience working with children/youth in out-of-home placement.
  • Experience with care coordination within child and family serving systems.
  • Training and experience in Wraparound.
  • Candidate must possess Colorado Wraparound Facilitator Credentialing or obtain it within the first twelve (12) months of employment.

Responsibilities

  • Conducts comprehensive interviews of youth and caregiver(s) to document social and emotional histories using the Child and Adolescent Needs and Strengths (CANS) or other specified tools to build consensus regarding the youth and family’s strengths and needs.
  • Participates in supervision with the Wraparound Supervisor on an ongoing basis to discuss professional goals, case progress and performance as well as to problem solve any performance needs or case challenges.
  • Cultivates trusting and reciprocal relationships with program participants, family members, formal and informal supports while also working to engage additional supports as necessary to create a cohesive and comprehensive Wraparound team.
  • Ensures that the youth and family/caregiver voice and choice, strengths, and cultural needs and preferences have a presence and priority in all case activities.
  • Works with participants and CFT members to develop plans of care according to the 10 principles of Wraparound which integrate systems mandates and the needs of the youth and caregiver(s) to reduce duplication of services and streamline complex plans.
  • Ensures that strategies, supports, and services paired with the youth and/or family/caregiver are associated with identified and agreed upon needs.
  • Leads the team in assessing the effectiveness of the plans and convenes meetings to modify plans as necessary.
  • Produces written meeting agendas, notes, Wraparound/Crisis/Prevention Plans and disseminates to members of the CFT.
  • Maintains a caseload of eight to ten families as assigned by the Wraparound Supervisor.
  • Keeps accurate and up-to-date documentation of all Wraparound activities in required data management systems.
  • Assists in the creation and updating of program policies, procedures, and guidelines.
  • Collaborates with participants, representatives of the Medicaid Regional Accountable Entity (RAE) and/or private insurance systems, Community Centered Boards (CCBs), School Districts and other professional stakeholders.
  • Works with Wraparound Supervisor to ensure that the correct system of care and correct funding streams are accessed.
  • Maintains ongoing communication with all team members and facilitates CFT Meetings, including Crisis Stabilization and Crisis Prevention Planning meetings to Wraparound model fidelity
  • Engages in effective meeting preparation conversations and activities to ensure productive meetings
  • Facilitates team problem solving aimed at eliminating or minimizing service gaps and barriers.
  • Participates in internal Wraparound agency meetings.
  • Performs as an active team member and when required, testifies in Court regarding participant/family progress and outcomes of Wraparound.
  • Follows all local, state and federal policies and procedures and adheres to strict confidentiality guidelines.
  • Represents the agency in a professional and knowledgeable manner while working to promote a visible and positive image of the agency.
