About The Position

The Heart Failure Program Coordinator is responsible for coordinating and facilitating all components of the heart failure program. This role directs the program’s Quality Improvement components necessary for accreditation and maintains communication with accreditation agencies. The coordinator ensures that the program is patient-centered, effective, and efficient, with the ultimate goal of optimizing patient care. Additionally, the role ensures the integration of other disease programming to manage chronic comorbid conditions and promotes collaborative care practices. This includes establishing partnerships with physicians and entities within the Reading Health System to provide identified related services.

Requirements

4 year Bachelor's Degree- OR- combination of relevant education & experience may be considered in lieu of Bachelor's Degree
PA Registered Nurse License
Specialty Certification Required within 6 Months of Hire

Cardiac experience preferred

Responsibilities

Coordinates and facilitates all components of the heart failure program.
Directs the program’s Quality Improvement components required to maintain accreditation standards and is responsible to maintain effective lines of communication between appropriate accreditation agencies.
Ensures that the programming is patient-centered, effective, and efficient.
Efforts are directed at optimizing the care received by the patient.
Ensures integration of other disease programming as appropriate to address chronic comorbid conditions.
Promotes collaborative care practices and participates in establishing partnerships with the physicians and the entities of the Reading Health System to provide identified related services.
