About The Position

The Heart Center Program Manager RN plays a vital role in the multidisciplinary team providing expert nursing care for patients with a broad range of complex cardiac conditions across the continuum of care. This position partners with cardiologists, cardiothoracic surgeons, intensivists, advanced practice providers, nurses and ancillary teams to ensure high-quality, patient centered cardiovascular care. The Program Manager serves as the primary liaison for patients, families, and healthcare providers, facilitating communication and ensuring seamless coordination of care in inpatient, procedural, post-discharge and outpatient settings. Responsibilities include managing patient navigation throughout the cardiac care journey, providing patient family education, supporting quality and regulatory initiatives, monitoring program outcomes, coordinating multidisciplinary care plans, and promoting evidence-based cardiovascular practices. The Program Manager is dedicated to improving outcomes, enhancing access to care, and helping patients achieve their highest level of cardiovascular health.

Requirements

  • Bachelors Degree (preferred)erably in Nursing or Health Care Related Field (required) Or Equivalent (required)
  • RN - Registered Nurse License - CA-BRN - California Board of Registered Nursing (required)
  • BLS - Basic Life Support - AHA - American Heart Association (required)
  • Minimum three (3) years Clinical, leadership and/or project management experience. (required)
  • Excellent communication skills
  • Ability to collaborate effectively across diverse teams to achieve high quality outcomes
  • Strong analytical and problem-solving skills
  • Leadership and project management abilities
  • Extensive advanced knowledge of nursing or respiratory care principles, clinical practice, quality and healthcare systems
  • Proficiency in the skills and knowledge required to perform the job in specialty area
