About The Position

The Hartford HealthCare Center for Healthy Aging is a resource and assessment center designed to make it easier for seniors, their loved ones and caregivers to access essential information and services to attain the optimal quality of life. The Healthy Aging Navigator is responsible for overseeing and managing care transition operations for older adults within an assigned region of the Hartford HealthCare system. Serves as a lead representative of the Hartford HealthCare Community Network within the region and is accountable for ensuring timely access to appropriate services, reducing avoidable readmissions, and improving consistency in transitions of care across clinical and community‑based settings. Supports operational planning, performance monitoring, and program expansion in alignment with organizational priorities. Key Areas include: Ownership & Oversight of Care Transition Program, Patient and Caregiver Engagement, Leadership Collaboration & Community Partnerships, Operational Analysis and Continual Improvement. Requirements

  • Bachelor’s Degree in Health & Human Service Field (Social work, Gerontology)
  • Five years working in Healthcare and Care Coordination
  • Five years working with older adults
  • Able to travel to locations throughout the region on a regular basis
  • Strong analytical skills, able to work with and interpret large amounts of data

Nice To Haves

  • Master’s Degree preferred
  • Exceptional Communication and Organizational skills

Responsibilities
