About The Position

Willow Valley Communities is seeking a full-time Healthcare Social Worker to support Residents and their families as part of an interdisciplinary care team. The role involves planning and implementing Resident care, completing MDS documentation, assessing psychosocial needs, developing interventions, and advocating for Residents throughout their healthcare journey. The position is located at Lakeside at Willow Valley Communities in Willow Street, Pennsylvania, and is scheduled Monday through Friday, 8:00 AM to 5:00 PM, for 40 hours per week. Starting pay is $23.00 per hour or more, based on experience.

Requirements

• Bachelor's degree in Social Work or a bachelor's degree in a human services field, including Sociology, Gerontology, Special Education, Rehabilitation Counseling, or Psychology.
• One year of supervised social work experience in a healthcare setting working directly with individuals.

Responsibilities

• Initiate Resident contact and conduct interviews and counseling to assess care needs.
• Listen and respond to Resident and family concerns, provide counseling regarding disease progression, and direct them to appropriate resources.
• Coordinate care plans upon admission and update them quarterly, annually, and when there are significant changes in condition, involving family members as needed.
• Establish, review, and update Social Services care plans and support plans.
• Oversee the Resident grievance procedure and assist with resolving Resident and family concerns.
• Address financial concerns of Residents and families and refer them to the business office as needed.
• Participate in care plan and support plan meetings with Residents and families.
• Coordinate resources to promote the highest possible level of functioning and facilitate discharge to the appropriate level of care.
• Collaborate with the Transition Coordinator and serve as a liaison between the facility and community organizations or hospitals.
• Coach team members through Resident concerns.
• Manage admissions and discharges, including notifications and paperwork, and facilitate onboarding meetings in skilled care.
• Coordinate additional Resident services as requested or indicated.
• Maintain discharge-planning and social-history documentation and documentation required by Department of Health and Department of Human Services regulations.
• Coordinate the Behavior Management Program and Psychotropic Medication Review meetings in skilled care.
• Coordinate psychiatrist, psychologist, eye care, and dental services.
• Educate new team members, Residents, and families on Resident rights, confidentiality, and the role of Social Services.
• Attend Town Meetings involving Residents, families, and volunteers.
• Complete and update Resident face sheets as needed.
• Participate in Continuing Care Assessment Team Meetings (CCAT).
• Complete cognitive and Level of Living Assessments as needed.
• Advocate for Resident rights and make referrals when appropriate.
• Provide data for Quality Improvement Committee initiatives and track Social Services quality indicators when necessary.
• Complete Social Services sections of MDS and maintain appropriate documentation in skilled care.
• Support Residents and families through end-of-life issues.
• Coordinate Resident placement in Supportive Living.
• Complete assigned Social Services Quality Indicators.
• Educate Residents and families on Medicare coverage, complete Medicare denial/cut letters when appropriate, and attend weekly Medicare meetings in skilled care.
• Develop and maintain objectives and standards to ensure Residents experience a clean, comfortable, orderly, and safe environment.
