About The Position

The Healthcare Outreach Manager is responsible for planning and implementing marketing and sales activities for WCCB’s Stone Rehabilitation Center and any direct admits for higher levels of care in Assisted Living, Memory Care or Skilled Nursing. Major functions include developing and maintaining referral sources, transition of care coordination, support of digital marketing campaigns to promote and communicate the services and mission of WCCB and the Stone Rehabilitation Center. Develops and maintains contact with key, targeted referral sources toward the generation of new referrals. Develops new referral sources and follows up with discharged clients to determine their need to re-establish services. Provides referral sources with current information on services. Requirements

  • Experience: 4+ years in Business Development and Referral Sourcing for Life Plan Communities or Senior Living.
  • Network: A proven "Book of Business" and established relationships with hospital Case Managers.
  • Tech Skills: Proficiency in CRM systems and Excel database management.
  • Marketing: Advanced experience creating, evaluating, and assessing strategic marketing plans.
  • Travel & Licensing: Requires 80% local travel; must have a valid VA driver’s license and proof of auto insurance.

  • Managed admissions processes specifically for skilled nursing facilities.
  • Proficient in Epic, Inquire, and Salesforce.
  • Gained hands-on clinical and operational experience within aging adult communities.

Responsibilities

  • Achieves individual sales and volume goals set by leadership.
  • Manages client referrals, intakes, and preadmission reviews for short-stay patients; fills in for the Admissions Director when needed.
  • Identifies community senior needs to plan, coordinate, and recommend educational programs and future service lines.
  • Develops the annual Marketing Plan and conducts ongoing market assessments, competitive analyses, and referral tracking to drive census growth.
  • Conducts ongoing medical and community outreach to educate about services.
  • Participates in the weekend on-call rotation for the Admissions Department.
  • Manages marketing material inventory and promotes the brand at on- and off-campus events.
  • Shares marketing initiatives in department planning meetings to coordinate team efforts.
  • Handles other duties as assigned.
