About The Position

Assists in the coordination of patient care services in the Allegheny Health Network Home Infusion pharmacy under the supervision of a licensed pharmacist consistent with federal regulations. This role involves providing excellent customer service, managing patient needs for supplies and equipment, conducting follow-up calls, and ensuring accurate documentation in the electronic health record. The coordinator plays a crucial role in maintaining the quality of infusion services by identifying and reporting patient issues to clinicians and coordinating the appropriate delivery methods for uninterrupted patient care. They also work with medical records technicians to obtain lab work and generate work orders, labels, and shipping documents under pharmacist supervision. The position requires adherence to legal and ethical guidelines, including protecting healthcare information, and involves training technician staff and maintaining records for staff requirements. Promoting the services positively and maintaining a professional attitude are key aspects of this role, along with a thorough understanding and adherence to the organization's mission, vision, and core values.

Requirements

  • Minimum High School diploma or equivalent
  • 6 months of pharmacy technician or related experience and a knowledge of pharmacy procedures, including parenteral admixtures
  • Knowledge of basic stability, compatibility, and storage requirements and understanding of reference materials available
  • Valid driver’s license and current automobile insurance

Responsibilities

  • Provide customers/patients with courteous, friendly, and efficient service and information.
  • Apply overall knowledge of customer/patient needs regarding supplies.
  • Assist patients/customers as required in obtaining appropriate equipment/supplies based on physician prescriptions and customer requirements.
  • Call patients/caregivers for post-admission follow up calls.
  • Complete documentation in electronic health record in a timely accurate manner.
  • Notify appropriate clinician with patient/caregiver issues to maintain quality of infusion services.
  • Determine appropriate delivery method to guarantee uninterrupted patient care.
  • Work with medical records technician to obtain lab work in a timely manner for clinical review.
  • Under pharmacist’s supervision, create work orders and generate labels for coordination of care for both new and ongoing patients.
  • Generate shipping documents with accurate delivery information, medication, supplies, and equipment.
  • Observe legal and ethical guidelines in performance of position and protect healthcare information according to Allegheny Health Network Home Infusion policies.
  • Train technician staff in regards to duties/responsibilities in the infusion pharmacy.
  • Maintain DME train and report/review staff annual requirement of completion.
  • Promote Allegheny Health Network Home Infusion Therapy Services in a positive manner and maintain a calm and professional attitude in the performance of this position.
