About The Position

This role is for a Healthcare Advocate with Clergy Care within the Archdiocese of Philadelphia. It is a mission-driven, full-time position focused on caring for priests, deacons, and religious by helping them navigate the healthcare system. Unlike bedside nursing roles, this position offers a slower pace, personal interaction, and the opportunity to build genuine relationships. The core mission is to ensure clergy receive the 'Right Care. Right Place. Right Time.' The role emphasizes providing personalized support, follow-up, and knowing the individual behind the medical chart.

Requirements

  • RN or LPN background strongly preferred (active license not required).
  • CNAs and Medical Assistants with 3+ years of healthcare experience are also welcome.
  • Exceptional communication and listening skills with ability to offer calm, compassionate support.
  • Highly organized, detail-oriented, and comfortable working independently.
  • Humble, service-oriented, and genuinely motivated by helping others.
  • Comfortable with light technology including Microsoft Office and remote monitoring tools.
  • Able to travel locally to member locations, facilities, and events.

Responsibilities

  • Build trusted, personal relationships with clergy through regular outreach and on-site visits.
  • Serve as a primary point of contact for healthcare navigation, benefits questions, and wellness needs.
  • Proactively identify unmet needs and connect members with services before problems escalate.
  • Provide health education and preventive wellness resources tailored to each member.
  • Coordinate medical, dental, and specialist appointments for seamless care experiences.
  • Assist members in navigating provider networks, benefits, and pre-authorization processes.
  • Connect members with community resources including transportation, home care, and support services.
  • Coordinate third-party services such as DME, home health, and specialty providers.
  • Conduct on-site hospital visits and serve as a personal advocate during hospitalizations.
  • Provide post-discharge follow-up and care coordination to prevent readmissions.
  • Visit members in assisted living facilities, skilled nursing facilities, and retirement homes.
  • Conduct home safety assessments and arrange modifications or meal services when needed.
  • Assist with remote monitoring tools including glucometers, blood pressure cuffs, and alert systems.
  • Collaborate closely with our broader support team: pharmacists, nutritionists, and social workers.
  • Partner with Archdiocesan leadership and affiliated ministries to ensure coordinated, comprehensive care.
  • Maintain accurate, timely documentation in accordance with HIPAA and PHI guidelines.
