The Health & Wellness Support Specialist will work as a team member, follow directions, and agree to supervision by the Nurse / Health Services Manager. This role will maintain and promote respectful and meaningful relationships with the residents. The specialist will help to ensure compliance with all State and County mandated regulations (6100s, 6400s, and 6500s) and provide leadership and model best direct care practices for new staff. This position involves administering medication, participating in medication training of new staff, and will become the primary med trainer once acclimated. The specialist will research and recommend training on specific healthcare needs and ensure all health-related Relias Trainings are uploaded in a timely manner. They will also help plan and conduct health-related activities, educate residents about medical appointments, and ensure documentation of refusals. The role includes assisting with eMAR configuration and oversight, reviewing eMARs for accuracy, acting as a liaison between PDC and Emmaus when the RN is not available, and assisting with resolving medication issues. The specialist will also assist in semi-annual physician order review and distribution, ensure accurate utilization of appointment tracking/health tracking features in Therap and other healthcare data entry, and attend medical appointments with residents as needed. They will ensure all follow-up is completed, including lab orders, paperwork filing, team communication, and scheduling future appointments. In emergency medical care situations, the specialist will work with the Point Person/Nurse to ensure documentation is collected, filed, and shared, and that follow-up is completed. This includes visiting individuals in hospital/rehab facilities, assisting with discharge planning, coordinating supplemental habilitation, and planning quality post-acute care. The specialist will track residents’ annual physical due dates and TDAP vaccinations, provide reminders, collect and review completed physicals, and forward them to the RN. Periodically, they will assist the agency nurse in reviewing medical books for completeness and accuracy. The role also includes participating in the On Call Rotation, attending staff meetings/trainings, assisting with other healthcare-related activities, working with the RN on Performance Based Contracting, and assisting with incident management/entering incidents into EIM, with training provided.
