Health & Wellness Coach CCBHC
About The Position
The Health & Wellness Coach serves as an integral member of the multidisciplinary team and a core part of the continuum of care for clients enrolled in Behavior & Primary Healthcare Coordination (BPHC). The coach provides individual, supportive coaching to improve clients’ physical health, coordinates care with internal and external providers, helps clients access and use resources, and promotes positive outcomes. The position provides life-skills training, care management, and structured interventions to help clients gain access to resources and develop skills to improve physical health, quality of life, and self-management of symptoms. Topics include health hygiene, lifestyle habits, nutrition, reducing sedentary behavior, and healthcare navigation. Support is provided across phases of care and adapts to the changing needs of the people served.
Requirements
- Bachelor of Arts or Bachelor of Science degree from a four-year college or university.
- Two years of direct-service experience working or volunteering in a human service organization.
- Effective written, verbal, in-person, and virtual communication skills.
- Customer service skills.
- Time-management skills.
- Analytical thinking skills.
Responsibilities
- Provide individual, supportive coaching to improve assigned clients’ physical health, including helping schedule medical appointments, communicate effectively with doctors, and follow medication instructions and other recommendations.
- Coordinate client care with internal and external providers and obtain Releases of Information for all medical providers involved in care coordination.
- Provide life-skills training, care management, and structured interventions to help clients access resources and build skills that support physical health, quality of life, and symptom self-management.
- Support clients with health hygiene, lifestyle habits, nutrition, reducing sedentary behavior, and healthcare navigation.
- Provide support across phases of care in response to the changing needs of the people served.
