About The Position

The Health & Wellness Case Manager is a key member of the Student Affairs team, providing coordinated support to students experiencing complex challenges that impact their academic success and well-being. This position manages a caseload of students of concern, delivers individualized interventions, and connects students with appropriate campus and community resources. The Case Manager supports student retention through crisis response, assistance with medical and mental health leave processes, and facilitating successful transitions back to the University, while collaborating closely with CARE teams and campus partners to ensure a coordinated, student-centered approach.

Requirements

  • Bachelor's degree in counseling, social work, higher education, public health, or a related field, plus one (1) year of experience supporting student populations in areas such as case management, counseling, residence life, advising, or related field or a combination of education and experience in which comparable knowledge and skills are acquired.
  • Demonstrated ability to manage sensitive situations with professionalism, discretion, and sound judgment.
  • Knowledge of student development theory, crisis intervention, and case management practices.
  • Understanding of mental health, wellness resources, and student support systems in higher education or community settings.
  • Strong interpersonal, communication, and problem-solving skills.
  • Ability to assess complex situations and coordinate appropriate interventions.
  • Ability to collaborate effectively across departments and with external partners.
  • Ability to maintain confidentiality and comply with FERPA, HIPAA (as applicable), and institutional policies.

Nice To Haves

  • Master’s degree in counseling, social work, higher education, public health, or a related field.
  • Licensed as a social worker or counselor.

Responsibilities

  • Manage a caseload of students experiencing elevated or complex concerns, including mental health, medical, academic, and personal challenges.
  • Conduct individualized assessments and develop tailored intervention and support plans.
  • Provide ongoing case management, including regular follow-up, monitoring, and documentation.
  • Facilitate referrals to campus and community resources, ensuring coordinated and timely support for students.
  • Serve as a resource to CARE teams and participate in threat assessment team processes, providing case insight and recommendations.
  • Support students in crisis, including those experiencing hospitalization, mental health emergencies, or significant life disruptions.
  • Provide consultation and guidance to faculty, staff, and families regarding students of concern.
  • Assist students in navigating the medical leave and return-to-campus process, including application support and re-entry planning.
  • Provide structured follow-up and support for students returning from leave to promote successful reintegration.
  • Develop targeted interventions, including check-ins, support groups, or other programming for at-risk student populations.
  • Collaborate with campus partners (Student Affairs, Academic Affairs, Residence Life, etc.) to support student success and retention.
  • Build and maintain relationships with off-campus providers and community resources.
  • Develop and deliver training and outreach to faculty and staff on identifying and supporting students of concern.
  • Promote student wellness through proactive programming, education, and communication efforts.
  • Maintain accurate case records in compliance with institutional policies and applicable federal and state regulations.
  • Analyze trends and data related to student concerns to inform intervention strategies and resource allocation.
  • Contribute to assessment efforts to evaluate program effectiveness and student outcomes.
  • Maintain a limited clinical caseload, as appropriate, to support students in crisis and contribute to continuity of care.
