About The Position

The HUC/CNA of the Extended Care Facility is responsible to perform both clinical and administrative duties of the nursing home. The role involves a combination of tasks including managing patient records, transcribing physician orders, ensuring a safe and comfortable environment, and providing direct patient care as a Certified Nursing Assistant.

Requirements

  • High School diploma or GED equivalency required.
  • CNA Certificate required.
  • BLS Required within 30 days of hire.
  • Basic computer skills in Microsoft Word/Excel and Outlook required.
  • Operates copy machine and fax machine.
  • Keyboard at 40 wpm.
  • Previous experience answering multi-line telephone system.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
  • Ability to communicate in visual, audio and/or written form.

Nice To Haves

  • Medical terminology preferred.
  • Prefer experience working on Meditech 6.0.

Responsibilities

  • Accurately transcribe physicians' orders to Pharmacy, Kardexes, Laboratory, Radiology, Rehab. Services, Home Health and others as needed.
  • Provide a safe, clean and comfortable environment for patients.
  • Complete admission, discharge and deceased resident paperwork.
  • Prioritize tasks and present problems to be handled during the shift.
  • Maintain patient's rights at all times.
  • Review charts to maintain physician orders, nurse’s notes and progress notes.
  • Write CNA assignments from schedule to the “brain”.
  • Develop BM list.
  • Receive and place confidential information with courtesy, accuracy and respect.
  • Address patients, visitors and co-workers in a pleasant, respectful manner and display courtesy at all times.
  • Adhere to the guidelines set forth in the facilities Personnel Policy Manual and Policy and Procedure Manuals.
  • Follow instructions received from licensed nursing staff (RN’s & LPN’s).
  • Adhere to uniform and hygiene requirements.
  • React properly and follow correct policy and procedure in an emergency and/or disaster.
  • Order supplies bi-weekly.
  • Prepare weekly bath/shower schedules.
  • Print, edit and file monthly resident flow records.
  • Process MAR’s for billing on a monthly basis.
  • Print, send to pharmacist for signature and file monthly drug regiment review.
  • Print, deliver for physician’s signature and file monthly physician’s orders.
  • On a monthly basis, remove and file old RFR, MAR and DRR.
  • Implement lab slips for monthly lab orders.
  • Provide monthly report to physicians for dates on progress notes.
  • Monthly ordering for Evergreen.
  • Print, edit and deliver schedules to Extended Care Director of Nursing.
  • Monthly schedule updates.
  • Monthly CNA assignments for evenings and nights.
  • Monthly Kardex review.
  • Complete annual review and keep CPR certification and TB vaccine current.
  • Wear Personal Protective Equipment (PPE) as situations require to maintain Infection Control standards.
  • Successfully complete the delivery of basic care needs for residents.
  • Perform personal hygiene and care of patients (oral care, hair care including shaving, nail care, peri care with or without attends, catheter care, AM/PM care, partial baths, complete baths, bed making, ambulation of resident, dressing residents, feeding including residents with swallowing difficulties, range of motion exercises, restraint use, hand washing).
  • Complete flow sheets and write brief statements and take vital signs.
  • Transfer patients to and from, either assisted or unassisted, with all the following: bed, wheelchair, chair, stretcher, examination table, commode, scales and floor.
  • Reposition patient if in a bed, wheelchair, chair, stretcher, examination table, commode, scales and floor.
  • Provide team support to all other residents in the unit as requested during the shift.
  • Provide direct care to partners (another CNA’s) residents during shift for breaks, lunches and with difficult residents.
  • Complete all necessary documentation of care given on assigned residents.
  • Report all necessary/abnormal information about the patient to the charge nurse.
  • Follow instructions received from licensed staff (RN’s, LPN’s, Physicians).
  • Participate in code situations as chest compressor or recorder in ECF, ACF or ED.
  • Address, with supporting documentation, any questions or concerns regarding patient care with the charge nurse.
  • Make frequent visual checks (at least every 30 minutes) on assigned residents.
  • Precept new employees.
  • Monitor the physical environment of the ECF and notify charge nurse when necessary.
  • Maintain required education credits per facility policy.
  • Submit to COVID testing as required by CMS.
  • Assist in other areas as the need arises.
  • Perform other duties as delegated.
