The Health Systems Navigator provides comprehensive, person-centered case management to participants in C@P’s supportive housing and Safe Parking programs. This position supports supportive housing residents in maintaining housing, health, and quality of life while supporting Safe Parking participants who are experiencing homelessness, in building stability and working toward permanent housing or other positive destinations. A primary focus of this position is health and healthcare navigation, particularly for supportive housing residents with complex or ongoing medical needs. The Navigator coordinates alongside healthcare, behavioral health, hospice, and community providers; helps participants navigate healthcare systems and benefits; and addresses health-related barriers that may impact housing stability.
