Requirements

  • High school diploma/GED.
  • Must be at least 21 years of age.
  • Valid IL drivers license, good driving record, auto insurance and reliable transportation required.
  • Must be able to pass pre-employment basic education test.

Nice To Haves

  • Post high school educational training in psychology, social work, special education, medical/hospital appointment and records, or human services and/or experience working with developmentally disabled individuals is desirable.

Responsibilities

  • Ensures that both adult and child individuals arrive at their scheduled medical and specialist appointments promptly by driving them, while also assisting them during the appointment process when necessary.
  • Successfully get on the DCFS group home license to ensure they are fully authorized to transport medications directly to the Children's Group Homes.
  • Assists the individuals to arrive promptly for scheduled medical and specialist appointments. This driving may include the individual in an agency vehicle or personal car.
  • Provides necessary direct support to each individual during the appointment to ensure that the client understands the doctor’s instructions. This includes but is not limited to staying with an individual while he/she is being seen by the physician and/or dentist.
  • Makes appropriate entries in the individuals’ case files as appropriate.
  • Provides verbal and written information to Nurse, Manager, and appropriate staff as needed to ensure appropriate follow-up treatment is offered to individuals, including but not limited to medication schedules and procedures required by physicians or dentists.
  • As needed, assists in arranging individuals appointments.
  • Attends or provides input at staff meetings and Community Support Team (CST) meetings as appropriate or necessary for individual clients.
  • Attends in-service and training programs as a scheduled supervisor.
  • Executes other duties assigned as required to assist in the provision of services within the CLS.
