About The Position

To ensure staffing levels are maintained per DHEC requirements for CNAs at Wesley Commons Health and Rehabilitation Center. Responsible for directly managing and oversight of all CNAs under the supervision of the Director of Nursing. Responsible for orientation, training, and proof of competency of all new and current CNAs. Infection Preventionist for facility. Other duties as assigned.

Requirements

  • Educational Nursing degree from an accredited school of nursing
  • Active license with the state of South Carolina
  • Maintains Red Cross CPR and First Aid Certification.
  • Must keep resident information confidential including diagnosis and medical history.
  • Must be able to properly handle body fluids.
  • Will be exposed to virus, disease and infection from patients and specimens in working environment.
  • Demonstrates verbal, non-verbal, and written and skills necessary for effective communication.
  • Demonstrates computer skills necessary to complete annual computer training modules.
  • Able to handle various emotions associated with residents and their families.
  • Able to relate information concerning a residents condition.
  • Able to walk or stand for extended periods of time.
  • Able to bend and squat.
  • Able to deal with confidential information of residents such as diagnosis and medical histories.
  • Ability to establish and maintain positive and productive working relationships with staff, co-workers, residents, families and volunteers.
  • Evidence of professional manner and conduct.
  • Optimal physical and emotional health.
  • Near and far visual acuity, sense of smell.
  • Able to frequently lift up 50 pounds and on occasion handle 75 pounds or more.

Responsibilities

  • Responsible for CNA orientation and training of new and current CNAs to include, 30/60/90 reviews, skill checks, and competency. Performs yearly reviews and competency for all CNAs.
  • Serves as a leader and professional role model for CNAs and licensed staff.
  • Maintain a professional appearance and greet all visitors and residents cordially and courteously.
  • Responsible for reviewing the daily staffing census sheets for accuracy and to ensure adequate staffing levels for CNAs in accordance with the current census/acuity level.
  • Serves as back up to the Nursing Services Coordinator for scheduling of all CNAs and Nurses of the HRC including assignment sheets.
  • Responsible for ensuring the CNAs accurately charge for resident supplies.
  • Interviews, selects, and recommends for hire CNAs for FT, PT, and PRN Positions. Submit requisition to Human Resources when positions are vacant.
  • Responsible for oversight of CNA documentation.
  • Responsible for making rounds daily to ensure that residents are appropriately groomed and dressed, beds made, room tidy, sheets are changed per policy and/or need and ensures exceptional resident care/services.
  • Serves as a liaison for the CNAs and nursing. Responsible for ensuring rounds are properly made and resident’s needs are met (ADLs – Pain, Potty, Positioning, Possessions) to include residents being well groomed, rooms tidy, etc.
  • Responsible for monitoring meals and meal services to ensure compliance with DHEC / QIS regulations.
  • Assist with new admissions as requested.
  • Assist the Director of Nursing with investigations to include submission of reports to DHEC Certification, Licensure, The Ombudsman and law enforcement.
  • Assist the Director of Nursing to resolve resident, family or visitor “Issues of Concern”. Responsible for investigating issues of concerns involving CNAs/care and providing documentation / follow up.
  • Provide assistance to the Director of Nursing to maintain educational files for nursing staff per DHEC requirements on employment, annual review, interactive in-service, and as needed.
  • Responsible for regularly scheduled staff meetings with CNAs. Must maintain a sign in sheet, minutes, and in-services/training reviewed.
  • Assist the Director of Nursing with other assignments as requested.
  • Responsible for the Infection Prevention Program including, monthly summary and providing in-services to all staff on infection prevention policies and procedures. Reporting at daily QM meetings and at quarterly QA meetings.
  • Responsible for attending care plan meetings and following up on any issues of concern related to the CNA’s care.
  • Responsible for completing 48-hour chart audits on all new admissions weekly and submitting the audits to the Director of Nursing/Administrator with findings/corrections.
  • Participate in manager call rotation. Serve as backup to review referrals when needed.
  • Adhere to all HIPAA guidelines and regulations.
  • Performs other duties as assigned.
  • Demonstrate knowledge and understanding of all policies and procedures and ability to reference them from appropriate books and materials.
  • Assist patients with emergency preparedness drills such as tornado/fire drills.
  • Special assignments as requested.
