About The Position
Supportive Concepts for Families (SCFF) is seeking a Health Services Coordinator to support our Health Services Department and help ensure individuals with intellectual and developmental disabilities (IDD) receive coordinated, high-quality, person-centered healthcare. This is a field-based nursing role where you'll work independently while collaborating with physicians, specialists, Direct Support Professionals, and interdisciplinary teams to ensure exceptional care for individuals with complex medical needs.
Requirements
- High school diploma or GED required
- Minimum 2 years of relevant healthcare administrative experience.
- Experience with electronic health records (EHR/EMR).
- Strong organizational, scheduling, communication, and follow-up skills.
- Knowledge of HIPAA, medical terminology, and healthcare documentation.
Nice To Haves
- healthcare-related associate degree preferred
- Experience supporting individuals with IDD is a plus!
Responsibilities
- Coordinate medical appointments, specialist referrals, and healthcare follow-ups.
- Maintain accurate electronic medical records and confidential health documentation.
- Support nursing teams with physician orders, clinical records, and medical information.
- Track physical examinations, laboratory results, immunizations, and preventive healthcare requirements.
- Communicate with physicians, pharmacies, hospitals, families, and residential teams.
- Support healthcare compliance, audits, reporting, and quality assurance.
