About The Position
Easterseals Florida is seeking a Community Health Resources Navigator to serve children and adults with disabilities in the Mascotte, Minneola, Groveland, and 4 Corners areas of South Lake County. This role involves providing health information and advocacy, making referrals to community resources, conducting disability screenings for young children, and recruiting clients. The Navigator will work closely with community partners to ensure clients have access to necessary health and disability services. The ideal candidate will be a current or former resident of Lake County, Florida.
Requirements
- Bachelor's degree in social work, education, or related field and four years of experience.
- Certification in Health Services (CNA, LPN, etc.) also considered.
- Experience/knowledge of the South Lake County (Mascotte, Minneola, Groveland and 4 Corners) social service network.
- Ability to work independently and effectively with minimal on-site supervision.
- Ability to meet approved driver qualifications to drive a personal vehicle for program-related needs (minimum age of 21 and good driving history).
- Must pass a Care Provider Background Screening.
Nice To Haves
- Bilingual (Spanish/English) preferred.
- Experience working with individuals with developmental disabilities a plus.
Responsibilities
- Provide health information and advocacy for children/adults with disabilities and their caregivers.
- Make referrals to community resources.
- Conduct disability screenings for children ages five and younger using the Ages & Stages Questionnaire (ASQ).
- Recruit clients for services.
- Work extensively with community partners to identify clients and ensure access to needed health and disability services.
- Coordinate and collaborate with South Lake County's neighborhood services, local community programs, childcare centers, and schools to obtain referrals.
- Integrate and participate in community events and committee membership within South Lake County and with community partners.
