About The Position

The Health Record Clerk is responsible for supporting all health record management activities for Family HealthCare Network. This role involves responding to requests for health and dental information, assisting patients and other requesters in accordance with federal, state, and FHCN guidelines, and ensuring HIPAA compliance. The clerk will manage authorizations for release of records, handle phone inquiries, and assist patients with record requests and form completion. They will also coordinate with outside copy services, verify patient demographic information, and maintain health record indexes and retrieval systems. Additionally, the position supports health care scanning and indexing, processes and stores health documents in the electronic health record system, performs quality reviews of scanned images, and forwards documents for provider review. The clerk will also update the Clinical Decision Support System based on consultation reports, retrieve documents from external systems, and resolve health record issues. Adherence to the Attendance and Absenteeism Policy and the ability to work at any FHCN location are also essential functions of this role. Other duties as assigned.

Requirements

  • Proof of completion of high school with a minimum overall GPA of 2.5 or completion of General Educational Development (GED) with a minimum overall score of 162.5.
  • If a higher-level degree was completed with a stronger GPA, proof of that GPA may be provided in lieu of the high school diploma GPA.
  • Minimum of two years of experience and knowledge of medical records, processes, and practices.
  • Ability to prepare basic correspondence and simple reports in Microsoft Word.
  • Ability to use Microsoft Excel to create tables and simple displays of information.
  • Ability to create basic presentations in Microsoft PowerPoint.
  • Effectively communicate routine or non-technical information to co-workers and others.
  • Effectively communicate written information (including electronic correspondence) and verbal presentations.

Responsibilities

  • Respond to requests for health and dental information.
  • Provide assistance to patients, family members, legal guardians/representatives, and referral sources regarding records requests or other information.
  • Receive authorizations for release of records and determine HIPAA compliance.
  • Return noncompliant authorizations to requesters.
  • Follow HIPAA privacy and confidentiality regulations at all times.
  • Respond to phone inquiries and in-person requests regarding the status of health records requests.
  • Assist patients with the completion of authorization forms for release of information.
  • Ensure requests are completed accurately and meet all FHCN and state-mandated requirements.
  • Coordinate with outside copy services and schedule copy/retrieval appointments.
  • Verify patient demographic information and facilitate corrections.
  • Maintain and operate a variety of health record indexes, storage, and retrieval systems.
  • Ensure subpoena requests are completed in accordance with required timeframes.
  • Collect fees for subpoenas and some medical releases and forward to finance daily.
  • Support health care scanning and indexing activities.
  • Process and properly store health documents in the electronic health record system.
  • Perform quality reviews of scanned images to ensure the integrity of information.
  • Forward scanned documents for review and signature by provider staff.
  • Close referrals and other orders based on received consultation reports.
  • Update Clinical Decision Support System based on received consultation reports.
  • Retrieve documents from hospitals’ records systems and transfer to FHCN’s electronic health record.
  • Address and resolve health records issues as they arise.
  • Verify authorization and prioritize records requests.
  • Problem-solve record locations while performing excellent customer service.
  • Include appropriate people in the decision-making process.
  • Make timely decisions.
  • Adhere to the Attendance and Absenteeism Policy.
  • Present to and work at any FHCN location based on business need.
  • Perform other duties as assigned.
